Part 01 · How it works

Mechanism.

Tirzepatide is a single engineered peptide that activates two gut hormone receptors at once: GLP-1 (the appetite-and-insulin one that semaglutide targets) and GIP (a second incretin hormone that helps the body handle fat and amplifies insulin release). Hitting both receptors with one molecule produces larger and more durable weight loss than targeting GLP-1 alone, which is how it pulls ahead of semaglutide in head-to-head data.

If semaglutide is a dimmer switch on hunger, tirzepatide is that same dimmer plus a second knob tuned to how your body stores and burns fat. Two levers pulled at once — not because it's stronger, but because the two signals reinforce each other.

GLP-1R agonism
Agonist at the GLP-1 receptor (class B GPCR) on β-cells and CNS satiety centers — glucose-dependent insulin secretion, delayed gastric emptying, appetite suppression.
GIPR agonism
Full agonist at the GIP receptor on β-cells and adipose tissue — amplifies incretin insulin secretion and modulates lipid storage; may enhance adipose insulin sensitivity via adiponectin.
Receptor balance
Balanced dual agonism (unlike selective GLP-1 agents); the GIP arm adds efficacy without proportional GI burden, explaining the favorable effect-to-side-effect ratio.
Half-life engineering
Fatty diacid side chain enables reversible albumin binding, yielding a ~5-day half-life consistent with once-weekly dosing.
CNS penetrance
Peripheral effects dominate, but both GLP-1R and GIPR signaling in hypothalamic feeding circuits contribute to the sustained reduction in 'food noise' patients report.

GI side effects more frequent than semaglutide

In direct comparison to semaglutide (SURPASS-2 head-to-head and indirect comparisons across SURMOUNT and STEP), tirzepatide showed numerically higher rates of nausea, vomiting, and discontinuation for GI intolerance — particularly during dose escalation past 10 mg.
What this means: The larger weight-loss effect comes at the cost of a wider side-effect window. Some patients tolerate semaglutide better; rotating between the two has no strong evidence base.

Same discontinuation regain dynamic as semaglutide

SURMOUNT-4 enrolled patients into a 36-week placebo-controlled withdrawal phase after open-label tirzepatide. Those switched to placebo regained an average of 14% of body weight; those continuing tirzepatide lost an additional 5.5%. The withdrawal effect is comparable to the semaglutide pattern.
What this means: Tirzepatide is also not a finite course. Plan for indefinite use, or for substantial regain after stopping — there is no published path to maintenance off-drug.

Lean mass loss observed in DEXA subgroups

In DEXA substudies, total lean mass declined alongside fat mass — proportional to weight loss, similar to semaglutide. No published RCT data yet show that adding resistance training reverses the body-composition profile during therapy.
What this means: Same caveat as semaglutide: older adults and patients with low baseline muscle mass should plan for resistance training and adequate protein during therapy.

Cardiovascular outcomes trial (SURPASS-CVOT) still pending

Unlike semaglutide (SUSTAIN-6, SELECT — both completed and positive), tirzepatide has no completed dedicated cardiovascular outcomes trial as of mid-2026. SURPASS-CVOT is expected to report in 2027. The cardiovascular signal is reasonable to infer from class effect, but is not yet directly demonstrated.
What this means: If the primary reason for considering tirzepatide is cardiovascular risk reduction in T2D, semaglutide currently has the dedicated trial evidence; tirzepatide is reasoning from class effect.
